In Order for your child to participate in QGlobal Programs, we need your consent and involvement in helping him or her have a productive experience and parent(s) or legal guardian(s) must be a QGlobal Club Member. Please read and sign this parental/guardian consent form if you would like QGlobal to continue the processes of considering your child to participate in our programs.

As parent and/or legal guardian of the above mentioned child, I authorize him/her to participate in the QGlobal Program(s). I hereby give permission for him/her to participate in that capacity, if
accepted by the QGlobal Community, I understand that he/she will be provided with training
necessary for safe and responsible performance of his/her trading training and that parents will
be expected to provide all the tools required for the child to participate in the program.

I authorize QGlobal to conduct all necessary screenings, trainings, and to contact the minor
directly (if possible) regarding those screenings and trainings in which the parent will be notified as well. I understand that the information obtained to determine program participants eligibility is
confidential and may be shared with the screening staff only in accordance with the applicable
federal, state, and local laws and FDCF Regulations.

I understand that he/she will not receive monetary compensation for participating in the program.

I understand that even when every reasonable precaution is taken, accidents can sometimes happen. Therefore, in exchange for QGlobal allowing my child to participate in this program, I understand and expressly acknowledge that I release the QGlobal and its members/leaders from all liability for any injury, loss, or damage connected in any way whatsoever to my child's participation. I understand that this release includes claims based on negligence, action, or inaction of QGlobal, its staff, directors, leaders. Furthermore, I understand that the QGlobal is not responsible personal property lost or stolen while my child is participating in QGlobal program.

I acknowledge the waiver set forth above. In accordance with the mission statement of QGlobal
Community, I am giving my child permission to engage in QGlobal program(s) and events.
